Registration Forms

It is the intent of St. Matthew Lutheran School to have as open an enrollment policy as possible. Students from all backgrounds are welcome and accepted; however, in order to keep peace and harmony within the school, some restrictions will apply.

1. Return the completed enrollment form to the Principal with the following enclosures:
• The non-refundable Instructional Material Fee payable to St. Matthew Lutheran School.
• A copy of your child’s most recent report card.
• A copy of your child’s birth certificate.
2. The parents/guardians and child will have an informal meeting with the school principal to discuss the child’s educational placement.
3. You will receive notification of your child’s enrollment status.
